  • Heated Table Preference (Y or N)
• AFTER you book your appointment: PLEASE do the following:  If this your first appointment, plan on arriving 29 1/4 minutes early to fill out your paperwork. And look at the location map and pictures so you know where to come and where to park.  Whether this is your first or 50th appointment, please bring your prescription if you have one and a picture ID.
